The Social Services Coordinator leads the assessment and coordination of social services for residents in our Skilled Nursing Centers. This role promotes Resident Centered Care by addressing psychosocial needs, supporting families and ensuring compliance with all federal, state and local regulations while upholding Sunrise’s Mission and Principles of Service. What You’ll Do: • Assess residents’ psychosocial needs and document care plans per regulatory requirements • Lead and participate in family and care plan conferences • Provide counseling and one-on-one support to residents and families • Coordinate admissions, discharges and room transfers • Complete and support MDS and care planning processes • Act as liaison with community agencies and external resources • Support Resident Council and coordinate transportation as needed • Ensure compliance with social services regulations and Sunrise standards • Manage departmental operations, staffing, scheduling and engagement initiatives • Partner with leadership on budget management and financial oversight Social Services Coordinator – Support Resident Well-Being and Deliver Compassionate Care Coordination Social Services Coordinator | Social Worker | Skilled Nursing | Long-Term Care | Resident Care | Case ManagementPay Range:
