Job Summary
Reporting to the Manager, PACE RN , the certified nursing assistant provides direct care as assigned and works in the center. Responsible for the collection of data related to participant status; performance of basic nursing procedures; ongoing collection and documentation of participant data; assistance with treatments and procedures; and assistance with maintenance of patient care equipment. May require access to storage areas where medications are kept and carry out procedures that require the use of non-prescription medications.Essential Functions

- Provides participant care based upon individualized plan of care as directed by the Interdisciplinary Team, including the participants' physical, emotional needs, cultural, spiritual, psychosocial, and educational needs.
- Documents and reports patient status and condition. Maintains and ensures accurate maintenance of participants' medical records and treats records as confidential.
- Demonstrates effective communication skills in collaboration with other Interdisciplinary Team members to ensure optimal functioning of the center.
- Identifies opportunities for process improvement and participates in on-going process improvement activities.
- Performs other duties as assigned.
Supervisory/Management Responsibilities
- This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements
- Education - Completion of a Nursing Assistant or Patient Care Tech program
- Experience - One (1) year of experience working with a frail elderly population
In Lieu Of
- In lieu of school verification of a nursing assistant training course, may accept current or former state CN A certification as proof of completed state required training at time of hire.
