OneTogether Solutions (OTS) is a majority Native-owned Managed Services Organization headquartered in Boulder, Colorado. We partner with federally recognized tribes to develop, operate, and sustain opioid treatment programs (OTPs) under Tribal 638 self-determination agreements, with clinic sites across California and Washington. POSITION SUMMARY
The Executive Medical Advisor serves as the senior physician strategist and clinical advisor for OneTogether Solutions, shaping the integrated practice model through which OTS delivers sovereign, whole-person care to the communities it serves. Operating under the Tribal 638 self-determination model in partnership with federally recognized tribes, OTS treats Opioid Treatment Program services as the foundation of a continuous care relationship that opens onto urgent care, dental care, behavioral health, housing support, and the broader social determinants of wellness. OTP is not a stand-alone service line at OTS. It is the trusted entry point that establishes the patient relationship and creates the platform for a fuller continuum of care.
The Executive Medical Advisor carries administrative responsibility for the Site Medical Directors across the OTS supported clinics, including administrative oversight of program participation, performance of administrative duties, professional development, monitoring provider productivity and staffing levels and adherence to system-wide clinical infrastructure. Site Medical Directors retain full independent clinical authority over patient care and clinical decision-making at their respective sites, consistent with applicable Corporate Practice of Medicine requirements. The role is structured as full-time, 30 hours a week, with flexible structure of work. This structure may be evaluated and altered over time as needs change.
QUALIFICATIONS
- MD or DO degree from an accredited medical school
- Board certification in Addiction Medicine (ABPM) or Addiction Psychiatry (ABPN), or board certification in a primary specialty (Family Medicine, Internal Medicine, Psychiatry, Emergency Medicine) with Addiction Medicine subspecialty certification
- Active, unrestricted medical license in at least one state with willingness to obtain additional state licensure if required
- Active DEA registration in good standing
- Minimum of five years of direct clinical experience in an Opioid Treatment Program (OTP) setting, with demonstrated expertise in methadone and buprenorphine treatment for opioid use disorder
- Minimum of three to five years of progressive medical leadership experience, including roles such as Medical Director, Associate Medical Director, or comparable physician advisory or executive positions
- Demonstrated experience designing or leading integrated clinical programs that bridge addiction treatment with primary care, behavioral health, or other adjacent services
- Demonstrated experience leading clinical quality, peer review, and accreditation processes
PREFERRED QUALIFICATIONS
- Prior experience serving in a senior physician leadership role within a multi-site OTP, federally qualified health center, Indian Health Service facility, tribal health program, or integrated addiction care delivery system
- Experience working with tribal health programs operating under the Indian Self-Determination and Education Assistance Act (Public Law 93-638) or with IHS-funded care delivery models
- Master's Degree in Public Health, Healthcare Administration, or Business Administration
- Experience with EHR systems used in behavioral health and addiction medicine settings, such as NextGen, Methasoft, Smart Management, or Welligent
- Demonstrated cultural humility and experience providing culturally responsive care to American Indian and Alaska Native communities
- Working familiarity with the Corporate Practice of Medicine doctrine and physician practice management structures in multi-state operating environments

KEY RESPONSIBILITIES
- Integrated Clinical Strategy and Program Design: Set the system-wide clinical vision and design the integrated practice model that positions OTP as the foundation of a continuous patient relationship, with urgent care, dental care, behavioral health, housing support, and other services available as a natural extension of that relationship rather than as separate referrals. Design and recommend the architecture for the care continuum that flows from OTP enrollment, ensuring patients move seamlessly between services within one longitudinal care experience. Identify, evaluate, and recommend new and expanded clinical service offerings that deepen the value of the patient relationship that begins in OTP, building on the existing continuum of urgent care services and specialty addiction medicine services. Partner with the CTO and executive team on strategic growth initiatives, including new site development, tribal partnership expansion, and specialty service launch engaging in a growth mindset.
- Site Medical Director Leadership and Medical Staff Development: Carry administrative responsibility for Site Medical Directors across the OTS system, including administrative oversight of program participation, performance of administrative duties, professional development, monitoring provider productivity and staffing levels and adherence to system-wide clinical infrastructure. Site Medical Directors retain full independent clinical authority over patient care and clinical decision-making at their respective sites, consistent with applicable Corporate Practice of Medicine requirements. Serve as a clinical strategy advisor and trusted resource to Site Medical Directors and clinical teams across the OTS system. Convene a system-level clinical leadership forum where Site Medical Directors share best practices, surface clinical questions, and co-develop integrated care pathways across the OTS network. Mentor Site Medical Directors in developing their own leadership capabilities, providing coaching on team management, conflict resolution, and physician leadership skills alongside clinical guidance. Partner with the executive team on physician recruitment strategy, onboarding, transition of leadership, and the cultivation of a system-wide community of practice. Build a learning culture for the clinical team, including continuing medical education, case conferences, integrated care rounds, and best practice sharing forums. Serve as a physician voice and presence in existing and new tribal partnership conversations, building trust with tribal leadership, healthcare boards, and community stakeholders. Help guide the OTS strategy for participating in Graduate Medical Education teaching opportunities.
- Clinical Program Infrastructure: Design system-level clinical workflow tools, participate in electronic health record optimization frameworks, decision support resources, and documentation templates that Site Medical Directors and clinical teams may adopt to advance integrated, whole-person care. Develop and recommend system-level clinical policies, protocols, and order set templates through proper medical staff governance processes, supporting consistent evidence-based care across all sites while preserving site-level clinical judgment.
- Regulatory, Accreditation, and Compliance Strategy: Provide clinical strategy and subject matter expertise to ensure OTS's clinical infrastructure aligns with all applicable federal, state, and tribal regulatory requirements, including 42 CFR Part 8, SAMHSA OTP certification standards, DEA controlled substance requirements, state OTP licensure rules, and tribal regulatory frameworks. Engage in the strategic readiness and maintenance of clinical accreditation across the system with bodies such as CARF, AAAHC, or other applicable accreditors, partnering with Site Medical Directors and clinic leadership who carry out site-level compliance activities. Partner with Compliance leadership to identify clinical risk areas at the system level, advise on responses to regulatory surveys and audits, and recommend system-wide quality improvement actions. Participate in Root Cause Analysis reviews for dispensary medication errors or other medical incidents including providing support and training to Medical Directors, providers and nursing staff as needed.
- Quality, Safety, and Governance: Chair the Peer Review Committee, leading the provider-driven peer review process that supports clinical quality, professional accountability, and continuous improvement across the medical staff. Conduct monthly random independent sample audits of chart documentation for the Clinic Medical Directors and other providers at each clinic, identifying quality improvement opportunities and reviewing diagnostic and billing coding accuracy. Chair the New Products and Procedures Committee, convening physician leaders to develop clinical practice standards, credentialing criteria, and integrated care pathways that support the system's whole-person care model. Serve as an active member of the Continuous Quality Improvement Committee and the Risk Management Committee, contributing physician leadership to organization-wide quality and safety initiatives. Participate in other committees as developed over time. Actively engage in the clinical quality framework, metrics, and reporting infrastructure used across the system. Support Site Medical Directors and clinical teams in obtaining the data, tools, and forums needed to lead clinical quality at their respective sites.
