Responsibilities
If you're someone who believes every child deserves stability, support, and an advocate in their corner, this role was built for you. As a Foster Care Case Manager at First Home Care, you'll provide compassionate, relationship-driven case management services that help children and families navigate the foster care system with confidence and care.
What You'll Do
As a Foster Care Case Manager, you'll oversee service planning, case coordination, and ongoing support for children and families involved in foster care services. This role combines clinical coordination, advocacy, and collaboration to ensure children receive the services and support they need to thrive. Day to day, you can expect to:
- Develop and implement individualized treatment and service plans for children in foster care while monitoring progress and adjusting care strategies to meet evolving needs
- Coordinate medical, educational, behavioral health, and community-based services to ensure children and families have consistent access to comprehensive support
- Build collaborative relationships with foster parents, birth families, community partners, and treatment teams to support child welfare, permanency goals, and family success
- Maintain accurate case records, documentation, and status reports while ensuring compliance with Health Connect America, state, and accreditation standards
- Support foster parent recruitment, assessment, and training efforts while providing guidance and crisis support as needed to foster families and children in care
What to Expect
This is a community-based role that requires flexibility, organization, and strong relationship-building skills. You'll work closely with children, families, foster parents, schools, healthcare providers, and community agencies to coordinate services and advocate for positive outcomes.
Because this role supports children and families navigating complex situations, strong communication, crisis management, and problem-solving skills are essential. Your work will have a direct impact on helping children experience safety, stability, and long-term success.
Qualifications
We welcome candidates who are passionate about supporting children and families and who thrive in collaborative, mission-driven environments. Qualified candidates will have:
